All new patients will need to complete these 3 forms (click on
the name to download and print the forms):
Please print out and complete these forms,
but do not sign them, as Dr. King will go over
them with you during your first meeting. Bringing these completed forms to your first
session will expedite the intake process.

Clients Who Are Minors |
|
Parents who are bringing in their child
(under 18) for IQ or LD assessments will complete paperwork
while at the office, so they should not complete forms prior
to their appointment. Also, please note that
parents are required to stay on the premises for the
entirety of their child's assessment. |
